Introduction

Interdisciplinary teams is an approach to care of the elderly patient in which different disciplines in the healthcare system physician, social workers, nursing, assistants, geriatric nurse practitioners, gerontological research analyst, collectively set goals and share resources and responsibilities to provide a state of the art care to the elderly.

The need of an interdisciplinary team is becoming more obvious with the growing aging population and the multiple problems that this group has in their health care. Patients who have complex medical, psychological, and social needs, a team approach is more effective in assessing the patient's needs and creating an effective care plan than are professionals working alone. Especially when it comes to the care of the elderly, this involves more time needed and each professional alone has limited time, but in combination they are able to provide the care and meet the needs of the elderly. This team approach also helps the caregivers to have more support and strengthen them as well.

Goals

In our today's society where we strive to achieve excellence in providing quality healthcare to our elderly, a team approach is a vital part in the practice of geriatric medicine. Helping the patients and caregivers understand and successfully go through significant changes in their lifestyle requires a lot of time and effort of several specialties working together.
